Australia Day 2020 - Nomination Form

The Georges River Council Australia Day Awards are presented annually to recognise and honour individuals who have significantly contributed to the Georges River community. Exceptional achievements in the areas of sport, environmental projects, arts and culture are recognised.

Nominate your community hero in one of the following categories:

  • Young Citizen of the Year
  • Volunteer of the Year
  • Sportsperson of the Year
  • Arts and Cultural Award
  • Green Advocate of the Year
 

An overall Citizen of the Year will be selected from the five categories above.

AWARDS PRESENTATION

Nominees will receive a formal letter confirming their nomination. Winners and nominators will receive invitations to the official award ceremony to be held on Australia Day, 26 January 2020 where winners will be announced.

NOMINATIONS

Nominees must be an Australia Citizen and live in the Georges River Local Government Area. All information contained in the nomination will be treated confidentially. Please follow the steps below when making a nomination:

  1. Complete the nomination form and select the relevant award category for your nomination.
  2. Provide a description of the nominee’s achievements. The description should address the selection guidelines and eligibility as outlined. You may either use the space provided or attach a separate page to the nomination. You can include supporting documentation and material such as photographs or brochures.
 

SUBMISSION DEADLINES

Send completed nomination forms to Georges River Council no later than 4:00pm, Friday 1 November 2019. Nominations postmarked after 1 November 2019 will not be accepted.

CATEGORIES


Young Citizen of the Year
This honour is awarded to a young person 25 years of age and under as of 26 January 2020 who has demonstrated a commitment to the betterment of the Georges River community, or given outstanding service to the community over a number of years.

Volunteer of the Year
This honour is awarded to a person who has shown outstanding dedication and contribution to one or more specific areas leading to the betterment of the Georges River community and provided their time voluntarily during the current year or given outstanding service to the community over a number of years.

Sportsperson of the Year
This honour is awarded to a Sportsperson either in the capacity of an athlete, coach, manager or administrator who has provided their time or raised the profile of their chosen sport during the current year, or shown outstanding achievement in sport over a number of years.

Arts and Cultural Award
This honour is awarded to a local person who has actively provided their time in contributing to the community’s arts and cultural identity during the current year, or provided outstanding service to the arts over a number of years.

Green Advocate of the Year Award
This honour is awarded to a local group or person who has supported a waste-smart, litter and plastic-free Georges River and contributed to the environmental awareness or preservation of local ecosystems during the current year or provided outstanding achievements over a number of years.

ELIGIBILITY CRITERIA

Please consider the selection criteria listed below when completing your nomination

  1. The nominee’s achievements must be consistent with the selection criteria outlined above.
  2. The achievements must have taken place within the Georges River Council area and be able to show community benefit and sustainability.
  3. Nominations must include the name and contact number of at least one independent referee who can provide supporting evidence for the nomination if required.
  4. Young Citizen of the Year must be under 25 years of age as at 26 January 2020.
  5. A candidate may only be nominated in one award category.
  6. Previous recipients of an award in the same category are not eligible for nomination.
  7. Elected nominees, previous recipients of an award in the same category, self-nominees or current Georges River Council employees are not eligible for nomination.

TERMS AND CONDITIONS

  1. By completing and submitting a nomination form, or formally accepting a nomination, each nominee agrees to be bound by the following terms and conditions of entry.
  2. The promoter is Georges River Council (Council), Corner MacMahon and Dora Streets Hurstville NSW 2220.
  3. All nominees must be, at their date of nomination, an Australian citizen and reside in the Georges River Council area.
  4. Nominees may only be nominated for one award category.
  5. All nominations must have a nominator and additional referee and include a response to the criteria on the nominee. All nominations must be made on the nomination form; nominations will not be accepted over the phone, via email, in letter form or via elected representatives.
  6. Elected nominees, previous recipients of an award in the same category, self-nominees or current Georges River Council employees are not eligible for nomination.
  7. Nominees for Young Citizen of the Year must be aged twenty-five (25) years or under as at 26 January 2020 and must specify their age to provide context around their accomplishments.
  8. Council reserves the right to accept, reject or refuse any nomination at any time.
  9. Council does not accept responsibility for lost, late or misdirected nominations.
  10. Council may vary the terms and conditions of this competition at any time at its absolute discretion.
  11. The 2020 Georges River Council Australia Day Awards includes five (5) categories, which include Young Citizen of the Year, Volunteer of the Year, Sportsperson of the Year, Arts and Culture Award and Green Advocate of the Year.
  12. The five Australia Day Award winners as well as the unsuccessful nominators will be notified prior to 26 January 2020 “Australia Day”. The overall Citizen of the Year will be announced at the official ceremony of Australia Day at Carss Bush Park.
  13. Prizes for each category include a certificate and medallion. The prize cannot be exchanged for cash or any other services.
  14. If there is any event that prevents or hinders Council’s conduct of the competition or Council’s ability to deliver the prize to the prize winner of the competition, Council may, at its discretion, cancel the competition and recommence it at another time under the same conditions or select another winner.
  15. All nominations are the property of Council and will not be returned to the entrants.
  16. Georges River Council is required under the Privacy and Personal Information Protection Act 1998 to collect, maintain and use your personal information in accordance with the Information Privacy Principles.
