Application Forms

To enrol your children at one of our centres, please complete our Wait List form.

We charge a non-refundable fee for each application. One application fee allows you to apply for all six of our early learning centres. You may apply for your preferred early learning centre or centres only. Your application can include more than one child from your family.

If our centres are full, your application joins the waiting list. We will contact you as soon as a place becomes available.

Immunisation

All children need immunisations to enrol in our early learning services. Before your child’s first day, you will have to provide:

• an Immunisation History Statement (IHS).

OR

• Australian Immunisation Register - Immunisation Medical Exemption form signed by a GP.
